Starting from November 1st, 2021, visitors receiving self-paid (out of pocket) Covid-19 PCR tests shall make appointment with and visit TSGH Travel Clinic, in case those visitors, at the same time, also apply for Covid-19 Vaccination Certificate/Record (Covid-19 Vaccination Yellow Book) or Airline Medical Clearance Form (Aviation Passengers’ Fitness for Flights Certificate), issued by Tri-Service General Hospital (TSGH), per formats regulated and published by Central Epidemic Control Center (CECC) of Taiwan Government.
